Harmony Hospice offers a full range of nursing care services and hospice resources to address the many special needs of your loved one during this difficult time. Our interdisciplinary approach embraces the following areas of expertise:
Hospice Nursing Services
Our nurses are with you every step of the way. Your care team is led by an RN Case Manager who coordinates all aspects of your loved one’s care plan. Our dedicated nurses choose to practice only hospice care. So they deliver not only specialized professional skills, but also the intangible sense of purpose and perspective that only a hospice nurse can bring.
Social Services
Our talented team of social workers handles the details so you can focus on your loved one. They participate in the initial assessment, help develop your care plan, assist with psychosocial care and manage discharge planning in both in-patient and home care settings. They also provide supportive counseling for patients and families coping with terminal illness and pool resources from the community to enhance quality of life throughout treatment.
Bereavement Services
Our trained Social Workers will provide bereavement “grief counseling” to you or your family members upon request. We offer this service because we believe that the family’s wellbeing goes hand-in-hand with the care and comfort of the patient.
Spiritual Care Services
Life-limiting illness is a time of profound change for people and their families. Spiritual care, from a broad spectrum of religious and spiritual traditions, is a core hospice service and is available to all Harmony Hospice patients and their loved ones.
Every person has a spiritual dimension--those non-material aspects of life represented in our relationships with family, friends or a greater power. Some people express spirituality through religion, others do not. We understand that spirituality is unique to each person and is an experience of finding one’s own way; an exploration of the “meaning of life.”
Anyone in the family may request spiritual counseling or a referral to community pastoral care.
Palliative Services
Palliative Care focuses on preventing and relieving the suffering of patients in all disease stages, including those undergoing treatment for curable illness and those living with chronic disease, as well as patients nearing end of life. Medications and treatments have a palliative effect if they relieve symptoms without having a curative effect on the underlying disease or cause. Palliative care helps relieve symptoms such as pain, shortness of breath, difficulty sleeping, fatigue, constipation, nausea, and loss of appetite.
