T
he Social Service component of Harmony Hospice is an integral part of the
Interdisciplinary team approach to the care of the terminally ill patient and
family. The Social Worker participates in the initial assessment, development
of the care plan, psychosocial treatment and discharge planning in both
in-patient and home care settings.
The Manager of the Social Services Department ensures that a complete
psychosocial assessment is completed upon admission. The Social Worker provides
ongoing supportive counseling for patients and families, as well as providing
resources from the community to enhance quality of life. Additionally, the
Social Worker assists in the initial assessment of bereavement risk and needs
in families.
The Social worker also participates in the Interdisciplinary Team Conferences
and acts as a support to team members and as liaison to community agencies. The
Social Worker provides education about Hospice and the Hospice Medicare benefit
to hospitals and other organizations, and as such, are "ambassadors" of Harmony
Hospice in the greater community.
It is the commitment of the Manager of the Social Services Department of
